During this visit, they will evaluate your health status, living environment, safety needs, and goals for recovery. This assessment helps us understand your unique situation and develop an appropriate care plan.
Step 3: Care Plan Development
Based on the assessment, our clinical team works with your physician to create a personalized care plan. This plan outlines the specific services you will receive, how often visits will occur, and measurable goals for your care. You and your family are important partners in this process.
Step 4: Ongoing Care and Communication

Step 5: Discharge and Transition
